In her magnificent essay Illness as Metaphor, Susan Sontag eloquently describes illness as “the night-side of life, a more onerous citizenship”. Robert McCrum, The Observer’s literary editor, has written movingly of an unseen “world of pain”. For my own small part, I have come increasingly to think about the “shadow world” of the long-term sick. Whatever words you use, they describe the same thing: a realm that exists alongside the everyday world, the world of the well; one that physically inhabits the same space. But a realm that is uncomfortably dark, where everything is flat and featureless, drained of colour. It is eerily silent, but listen carefully and you will hear the cries of untold anguish with which it is filled.
